At Bekem Community Care, we provide a range of services designed to provide each client with the highest level of comfort. At Bekem Community Care, we provide the following services and treatments.
Periodic visits by social workers and registered nurses to monitor medical conditions , evaluate care services and offers support and training. It is supposed to be social workers instead of special workers.
24 hours / 7 days coverage for emergencies.
Long term services program that supports and enables individuals who require assistance with activities of daily living to maintain their dignity and pursue typical life routines with individualized assistance from a family or non-family caregiver.
Active supervision and assistance to ensure the member’s health and well-being.
Medication reminders, scheduling and provision of transportation to appointments.
Eligibility
- Prospective members must be over the age of 16 and have MassHealth.
- Prospective caregivers must be over the age of 18.
- Prospective member’s must need assistance with Activities of Daily Living (ADL’S) which are the tasks of everyday life such as eating, dressing, getting into or out of bed or chair, taking a bath or shower, transfers, walking and using the toilet.
